Dated "4/8/86," the two-sided card measures 6" x 8", gives her agent's name and address for her account address, her birth date "8/15/58," names her birth place at Bay City, Michigan and her mother's maiden name, "Madonna Fortin." In very good condition with a few very small stains, one on the front next to her signature and three on the verso. Estimate $400-$600.
