This is the official master set of these multi-page application forms all containing the Australia Coat of Arms at the top left corner of the first page -- All personal information was filled out in each band members real name and furthermore, were also SIGNED by each band member in their real name -- The Gene, Ace, and Peter multi-page applications are complete and SIGNED.
