This is Bernie Madoff's original birth certificate from Beth Israel Hospital in New York City. The certificate is dated April 29, 1938 and is signed by the physician in charge and the medical director of the hospital. It states Madoff's full legal name, "Bernard Lawrence Madoff" and gives his father's name as "Ralph Madoff" and his mother's maiden name, "Sylvia R. Muntner" with their birthdates, place of birth (U.S.A.) and place (U.S.A.) and date of marriage. The certificate bears the Beth Israel Hospital official gold seal. On the back are Madoff's newborn footprints, one for his left foot and one for his right foot. His mother's left and right thumb prints are also presented. His sex, male, and birth weight 7 lbs 8 oz are listed as well as the certificate's serial number, 3452. Measures 11 x 8.5.
