Tupac Shakur Twice Signed Screen Actors Guild Dental Expense Claim Form

Tupac Shakur's twice-signed dental expense claim form from the health plan he received from the Screen Actor's Guild (SAG) of which he was a member. With the Screen Actors Guild Producers Health Plan stamp on the upper left corner, dental expense claim form from MetLife, was submitted by Deathrow Records. Tupac has signed the form twice in green ink and it is fax dated at the top "2-1-96". Measures 8.5 x 11. Very good condition. From the personal collection of a very close friend of Tupac's, who was considered part of their family, the set list comes with a letter of provenance from this individual. Also comes with a Gotta Have Rock & Roll™ Certificate of Authenticity.
