Madonna filled out this two-page application to the Art Worlds Institute of Creative Arts in Ann Arbor Michigan in her own hand, writing her name as well. Madonna was an 18 year old student at the Institute before moving to New York City. While Madonna attended the school she modelled for the earliest known nude photographs taken of her. Handwritten in black ballpoint pen, the application measures 8.5 x 11. The social security number has been covered for the purposed of showing the document online, but appears on the actual document. In very good condition, the application was torn in one section and has been mended with tape. The application has been laminated. Comes with a Gotta Have Rock & Roll™ Certificate of Authenticity.
