Madonna has signed her name "Madonna Ciccone" to this Employee's Claim for Workers' Compensation Benefits form in black ink. The form is filled out either in Madonna's hand or the hand of her long-time assistant Melissa Crow, but is signed by Madonna. The document originally came from Melissa Crow's personal collection. The claim was for an injury she sustained while filming "A League of their Own" at Wrigley Field in Chicago. The scene for the try-outs for the team were shot at Wrigley. Madonna has filled out the form in he own hand and has signed it as well, dating the form "8/2/91". The date of the injury was a few days before this, and her description of the injury is faint, although the words "Sore scratchy throat" seem to come first, after which there is more. During the filming of "A League of Their Own" there were many injuries to the actresses in the film as they had to wear skirts as their uniform and there was a lot of sliding into base, etc., as part of the game! Measures 8.5 x 11. Comes with a letter of provenance and a Gotta Have Rock & Roll™ Certificate of Authenticity.
